COMMUNICATION CONSENT Because we desire to follow Jesus together all 168 hours a week, the amount of communication between your student(s) and their leader(s) may increase. For the protection of students and leaders, we would like your consent as a parent to communicate with your student on an online or digital platform. You have the role of creating parental boundaries and controls and we respect your family’s decision. We also recognize that our role is to partner with you. As such, our youth leaders will be communicating more with you and will be recording this information for our records and the safety of your youth. Please be aware that we will be asking for permission more often for events and a written reply will be necessary. The communication platforms that may be used include but are not limited to: TikTok Instagram Facebook Messenger Discord Zoom Text Whats App Phone calls [not after 8pm] Our leaders will communicate with your students for the purpose of: Sharing Information Event Planning Checking In Group Conversations Group Discipleship I give consent for my student(s) to be communicated with via these platforms during the youth year 2022-2023 MEDICAL CONSENT I release Central Heights Church and its Youth Staff from any liabilities incurred by my child while at CHC Youth. In the event my child suffers sudden illness, accident, or injury, I give permission for any emergency treatment that is deemed necessary by a licensed physician. Every effort will be made to contact the parent or guardian prior to any emergency treatment.
